Moshir Jacob, MD, CPE, CMD

Dr. Jacob completed his internship and residency at the University of Toledo Medical Center (formerly the Medical College of Ohio) and has a rich academic history, having served as a clinical assistant professor at the University. Presently, he holds the esteemed position of Clinical Assistant Professor at Central Michigan University. He is dual board-certified in Internal Medicine and Obesity Medicine. Dr. Jacob has earned the distinguished title of Certified Physician Executive, awarded by the American Association of Physician Leadership. Additionally, he holds a certification as a Certified Medical Director, having earned his credentials from the American Board of Post-Acute and Long-Term Care Medicine. Dr. Jacob currently holds the esteemed roles as the Chief Medical Officer of both the Toledo Clinic and Independence Health ACO, demonstrating his expertise, proficiency, and leadership in the healthcare field. He is dedicated to optimizing the longevity and wellness of the entire family. Dr. Jacob is a member of the International Society on Aging and Disease, the British Society for Research on Aging, the International Longevity Alliance, and the Wellness Longevity Academy.

Now a Clinical Assistant Professor at Central Michigan University, Dr. Jacob embodies continuous improvement and lifelong learning. With dual board certifications in Internal Medicine and Obesity Medicine, his expertise is unmatched.

As a Certified Physician Executive by the American Association of Physician Leadership and a Certified Medical Director by the American Board of Post-Acute and Long-Term Care Medicine, Dr. Jacob’s leadership and management skills within the healthcare sector have positively impacted The Toledo Clinic and Independence Health ACO as the Chief Medical Officer.
