Dr. Moshir Jacob is a board-certified internal medicine physician in Toledo, Ohio who provides primary care for adults of every age, from young adults to seniors. What is an internist?

An internist is a physician trained specifically in adult medicine. Internists are often called “doctors for adults” because they are trained to handle complex problems and several conditions at once, which is exactly what most adults need as they get older.

Primary care for every adult

Primary care is for everyone, not just one age group. Whether you need your first adult doctor, have just moved to the area, are turning 65 or are losing a long-time physician to retirement, our practice makes the change easy. We help with the paperwork of switching, review every medication you take and bring your preventive care up to date from the first visit.

How we manage long-term conditions

Chronic conditions rarely travel alone. Diabetes raises heart and kidney risk; high blood pressure and high cholesterol add to it; weight and sleep affect all three. Instead of treating each number in isolation, Dr. Jacob looks at the whole picture and sets clear targets with you.

The goal is simple: fewer emergencies, fewer hospital stays and more good years.

More healthy years, not just more years

As patients get older, Dr. Jacob’s goal is health span, not just life span: staying strong, clear-thinking and independent for as long as possible. That shapes every visit. Beyond treating today’s problems, he looks for what threatens your independence tomorrow, including muscle loss, falls, poorly controlled blood sugar or blood pressure, too many medications, poor sleep and isolation, and acts on it early.

Dr. Jacob also shares practical, research-based advice on healthy aging through his free health guides and The Weekly Health Mile newsletter.

Preventive care and annual physicals

Good primary care catches problems early. Your visits include age-appropriate screenings, immunizations, lab work and practical guidance on diet, activity, sleep and stress. Results from our on-site lab are usually ready within a day or two and are shared through the patient portal after your provider reviews them.

Wellness visits and Medicare services

Care after a hospital or rehab stay

The weeks after a hospital stay are when problems are most likely. Our team contacts you soon after discharge, reconciles new and old medications, and sees you promptly for a follow-up visit. Dr. Jacob also has long experience in rehabilitation and long-term care settings, so the handoff back home is smoother and safer.

Telemedicine and same-day sick visits

Established patients can use phone or video visits for follow-ups, medication questions and many minor problems, through Dr. Jacob’s secure online waiting room. When you are sick, same-day appointments are available, so you can often avoid urgent care or the emergency room.

One campus, many specialists

Our office is part of The Toledo Clinic at 4235 Secor Road, a network of more than 265 independent physicians and health professionals across 40 specialties. When you need a cardiologist, lung specialist or other specialist, Dr. Jacob can usually refer you to someone in the same building, along with lab, imaging, physical therapy and pharmacy services. Free valet parking is available Monday to Friday.

Your care team

You may see Dr. Jacob or one of the nurse practitioners he trained, all working as one team with shared records and shared standards. Meet the team on our About Us page.

Switching from another primary care doctor

Switching is simpler than most people expect. Call us or use our Switch Your PCP page, and we will help request your records. If you have a Medicare Advantage HMO, your plan may need you to name Dr. Jacob as your PCP; our staff can walk you through it.

Frequently asked questions

Is there a difference between internal medicine and family medicine?

Family doctors care for all ages, including children. Internists focus only on adults and are trained in depth to manage complex and multiple chronic conditions.

How soon can I be seen as a new patient?

New patients are usually seen within a week, and same-day sick visits are available.

Do you accept my insurance?

We accept Medicare, Medicare Advantage and most major commercial plans, including Medical Mutual, Anthem Blue Cross Blue Shield, Aetna, UnitedHealthcare, Humana and Cigna. Call (419) 479-5728 to confirm your plan.

What is the difference between the Welcome to Medicare visit and the Annual Wellness Visit?

The Welcome to Medicare visit is a one-time preventive visit in your first year of Part B. The Annual Wellness Visit is a yearly planning visit after that. Both focus on prevention and are covered without a copay when the eligibility rules are met.

How often should I see my doctor if I have several chronic conditions?

Patients with multiple chronic conditions often do best with visits several times a year. Dr. Jacob will recommend a schedule that fits your health.

Can I contact the office after hours?

Yes. There is an after-hours line for urgent calls, and Dr. Jacob prefers that you call it first, because it gives you direct access to him and his team after hours when you need it. For a life-threatening emergency, call 911.

Do you see patients from Sylvania and Michigan?

Yes. Patients come from across Greater Toledo, including Sylvania, Maumee and Perrysburg, and from Southeast Michigan.
