PrimeYears App Privacy Policy

PrimeYears (“the App”) is the official mobile application of the medical practice of Moshir Jacob, MD at The Toledo Clinic, 4235 Secor Rd, Toledo, OH 43623 (“we,” “our”). This policy explains what information the App handles.

The App Does Not Collect Personal Data

PrimeYears does not collect, store, or transmit any personal information, health information, usage analytics, or advertising identifiers. The App requires no account, no login, and no registration. All educational content in the App is stored on your device.

Third-Party Services You May Choose to Use

The App contains links that open separate, independent services. When you use them, you are subject to those services’ own privacy policies, not this one:

• healow (appointment booking) — operated by eClinicalWorks. Any information you enter is governed by healow’s privacy policy and our practice’s HIPAA-compliant records systems.
• doxy.me (telehealth visits) — a HIPAA-compliant telemedicine platform governed by doxy.me’s privacy policy.
• TikTok (educational videos) — video content is displayed from or linked to TikTok and is governed by TikTok’s privacy policy.
• Phone, email, and maps links — open the corresponding apps on your device.

Protected Health Information (PHI)

The App is not a medical records system and does not access, store, or transmit PHI. All patient records, appointment details, and telehealth communications are handled exclusively within our HIPAA-compliant systems (healow / eClinicalWorks and doxy.me), not within the App.

Children’s Privacy

The App is intended for adults seeking information about our practice. We do not knowingly collect any information from anyone, including children under 13.

Changes to This Policy

If a future version of the App changes what data is handled, we will update this page and the effective date above.
