Choosing a 2027 Medicare plan? Choose your doctor first.

Dr. Jacob accepts Original Medicare, Medicare Supplement plans and Medicare Advantage plans. Book your first visit now and keep the same doctor when your new plan starts January 1.

Before you pick a plan, check three things:

  1. Is your doctor in network? Call (419) 479-5728 and we will tell you.
  2. Are your prescriptions covered?
  3. If an HMO requires you to name a primary care doctor, name Dr. Moshir Jacob.

No waitlist. No call center. A member of our own staff answers the phone and books new patients this week.

Primary care that welcomes Medicare patients

Turning 65, retiring, changing plans or losing a long-time doctor is a common time to look for a new primary care physician. Our practice is set up for exactly that, and we care for adults of every age. We handle the paperwork side of switching, review every medication you take, and make sure your preventive care is up to date from the first visit.

More healthy years, not just more years

Dr. Jacob’s goal for every older patient is health span, not just life span: staying strong, clear-thinking and independent for as long as possible. That shapes every visit. Beyond treating today’s problems, he looks for what threatens your independence tomorrow, including muscle loss, falls, poorly controlled blood sugar or blood pressure, too many medications, poor sleep and isolation, and acts on it early.

Dr. Jacob also shares practical, research-based advice on healthy aging through his free health guides and The Weekly Health Mile newsletter.

Dr. Jacob also has long experience in rehabilitation and long-term care settings, so the handoff back home is smoother and safer.

Switching from another primary care doctor
